Participant Waiver & Acknowledgement

I voluntarily agree to participate in the Soccer Cage Atlanta 404 Tournament ("Tournament") to be held on Saturday, August 16, 2026 at a privately owned location in Dacula, Georgia.

I understand that my participation is entirely voluntary and that soccer and recreational athletic activities involve inherent and significant risks. I acknowledge and fully accept all risks associated with participation, including but not limited to sprains, strains, fractures, muscle injuries, concussions, traumatic brain injuries, collisions with other participants or structures, slips, falls, field or surface conditions, heat-related illness, dehydration, permanent disability, paralysis, or death.

I understand that these risks may arise from my own actions or inactions, the actions of others, field conditions, equipment, weather, or the condition of the private property where the Tournament is held. I certify that I am physically capable of participating and assume full responsibility for my own health, safety, and well-being before, during, and after the Tournament.

RELEASE OF LIABILITY

In consideration for being permitted to participate, I hereby release, waive, discharge, and hold harmless Soccer Cage Atlanta, the tournament organizers, coordinators, volunteers, referees, sponsors, affiliates, property owner(s), employees, agents, representatives, successors, assigns, and fellow participants (collectively, the "Released Parties") from any and all claims, liabilities, demands, damages, losses, expenses, or causes of action arising out of or related to my participation in the Tournament, including but not limited to personal injury, illness, property damage, loss of personal belongings, disability, or death, whether caused by negligence or otherwise, to the fullest extent permitted under Georgia law.

This release also includes any claims arising from travel to and from the Tournament.

PRIVATE PROPERTY ACKNOWLEDGMENT

I understand that the Tournament is being held on privately owned property. I agree to enter and use the premises entirely at my own risk.

I accept full responsibility for any injuries, accidents, or damages to myself or my personal property occurring on or around the premises, including parking areas and common spaces. I further agree to be financially responsible for any damage I cause to the property or Tournament equipment.

PARTICIPANT CONDUCT

I agree to:

• Follow all Tournament rules and decisions made by Soccer Cage Atlanta staff and referees.

• Conduct myself in a respectful and sportsmanlike manner.

• Refrain from fighting, abusive language, violent conduct, or dangerous play.

• Accept that Soccer Cage Atlanta reserves the right to remove any participant for misconduct without refund.

MEDICAL AUTHORIZATION

In the event of an emergency, I authorize Soccer Cage Atlanta to obtain emergency medical treatment on my behalf if I am unable to do so. I understand that I am solely responsible for all medical expenses incurred.

PHOTO & VIDEO RELEASE

I understand that photographs and videos may be taken during the Tournament. I grant Soccer Cage Atlanta permission to use my name, image, likeness, voice, and photographs for promotional, advertising, social media, and marketing purposes without compensation.

ACKNOWLEDGMENT

By signing below, I acknowledge that:

• I have carefully read and fully understand this Waiver and Release.

• I understand that I am giving up certain legal rights, including the right to sue.

• I voluntarily agree to all terms contained herein.
